An employee was greasing the tooling on a punch press while manually rotating the punches. His finger was caught along the top of the punches, resulting in an a partial amputation of the right index finger.
An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool.
