An employee was on a ladder, adjusting the upper limit switch for an ash expeller. He slipped off the ladder and his left ring finger was caught between the expeller piping and the moving ram cylinder, resulting in a finger fracture and an amputated fingertip.

An employee stood on a conveyor wall to unclog the cotton feeder. The employee fell on the moving conveyor and fractured his right arm. The employee was hospitalized.

Multiple employees were using baling buckets to remove 750 gallons of 150-degree hydraulic oil out of a pit in a confined space for approximately 5.5 hours. The injured employee was standing on a ladder performing this task when they fell feet-first into the hydraulic oil pit, resulting in burns on both legs.

An employee was climbing down a ladder. The ladder was unsecured and the employee fell, severely cutting his right arm on metal siding. He was hospitalized and required surgery.

An electrician was on a 9-foot scaffold performing valve maintenance on an 8-inch line near a heat exchanger. The electrician was working to cycle a valve that failed to close. Due to a misaligned drain line and an open manual valve, hot liquid dextrose (~250 F) was released onto the floor below, creating a steam cloud. The electrician was evacuating the scaffold using the scaffold ladder when they slipped and fell 5-6 feet into the hot liquid, sustaining first- and second-degree burns to multiple parts of their body including both arms, both hands, their back, and their buttocks.

An employee was operating a horizontal belt sanding machine that rotates a metal sanding belt. The employee switched hands while the belt transitioned from left to right. When the machine's speed was adjusted, the employee s left hand was pulled into the machine and crushed. The employee was hospitalized and required surgery.

An X-ray machine had lost power and an employee went to replace the power supply. While replacing a cable on the power supply assembly, the employee's right hand contacted the grounding terminals coming out of the back of the power supply assembly and they received an electric shock. The employee was hospitalized.

On March 19, 2025, an employee was retrieving a medical specimen from the back of a vehicle. When he turned to enter the building, he either tripped over something or lost his footing and fell due to an incline in the pavement. The employee was hospitalized with a head abrasion and injuries to his right shoulder and right hip.

Employees were pulling a chemical sample from a device (sample port). The sample wouldn't come out because there were two samples in the port. The employees then turned off and deenergized the machine to 0 pounds per square inch. They unscrewed the top of the machine to retrieve the stuck samples when a small amount of pressure caused liquid to squirt out and onto one of the employee's face shields as the lid was removed. The employee was hospitalized due to chemical exposure.

An employee was venting a liquefied natural gas cylinder. Static electricity from a plastic garbage bag on the ground ignited the gas, causing a flash fire. The employee suffered second-degree burns to the hands and face.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
