The employee pushed one side of the dolly while his co-worker pushed on the other side of the dolly. The employee lost his footing and fell. The dolly then fell on top of him and the handle pierced his right foot through the tongue of his shoe, resulting in a puncture wound.

An employee was getting onto a pallet racking system. The employee's right foot went through the racking mesh, which lacerated the employee's leg.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
