A mechanic was performing preventative maintenance on a robot. The employee noticed the right grease port next to the pallet pusher bar was blocked, so they notified the operator. The employee moved to the next grease port as the operator moved the pusher bar. The pusher bar then moved on both the left and right side in tandem, causing it to contact the mechanics right middle finger at the grease port hole. The employee suffered a middle fingertip amputation and a fracture to the tip of the bone.

The employee's safety toecap of his boot got caught in a 4-inch-wide gap separating sections of the conveyor rollers, causing him to twist his leg and fall.
