An employee was removing straps from a load of cardboard on a truck. A strap broke causing the pile of cardboard to collapse and knock the employee off the truck. The employee fell to the ground, fracturing his back.

An employee was using a rear load trash truck to empty an 800-pound cardboard collection dumpster. The dumpster over tilted and then rested against the truck. The employee pulled the dumpster back creating a free fall until the dumpster was caught by the lifting cable. The dumpster struck the employee on the left shoulder before coming to rest on the cable, resulting in a shoulder fracture and dislocation. The employee was hospitalized.
