An employee was operating a band saw on the fabrication floor and suffered an amputation to their right index finger.
An employee was cleaning equipment. The employee came into contact with a sprocket on a conveyor, which deeply lacerated the employee's upper right arm.
An employee was cleaning the floor and placing material into the auger system for processing when his protective clothing and foot became entangled in the rotating auger shaft. The employee's leg was crushed, resulting in a laceration and a toe amputation.
The injured employee was driving a pallet jack. He rounded a corner as another employee on a stand-up reach truck was shifting into reverse to back up. The two vehicles collided and the pallet jack was swept into stationary racking used for storing product, causing the pallet jack driver to become pinned against the racking. When the employee grabbed the loose chain, a light sensor caused the machine to restart. This created a pinch point between the chain and sprocket that caught the employee's left little finger. The finger was surgically amputated at the second knuckle.
