An employee was carrying a box inside a warehouse when it slipped out of their hands and struck their knee. The employee then fell and landed on the concrete floor, resulting in a right femur fracture that required surgery and hospitalization.

An employee was walking from a palletizer to the production line conveyor to get a case of water to finish off a damaged pallet. He tripped on an uneven part of the floor (a drain that was 3/8-inch deep, 5-inches wide, and 5-feet long) and fell sustaining a broken left femur.

A forklift hit an employee. The rack on its forks pinned him against a truck, and he suffered two broken ribs and a punctured lung.

An employee was adjusting a forklift's fork extender chain. The chain lacerated three fingers on the employee's right hand. He was hospitalized, undergoing surgery.

An employee was retrieving a bottle cap from a de-capper machine. The mechanism caught the employee's right hand, lacerating it. The employee was hospitalized.

An employee was operating a machine when a wire pulled his right glove resulting in amputation of the index and middle fingertips.

An employee was grinding chicken when their right index fingertip was caught in the grinder and amputated.

An employee was sharpening peeler blades when their shirt was caught in the grinding wheel. The employee sustained tendon damage and a crushed right hand.

An employee was passing through a large metal door when the door closed on the employee, resulting in a cracked rib.

An employee was preparing to cut lumber on a panel saw when the clamp engaged and caught the employee's left middle finger, resulting in a fingertip amputation.

A mechanic was performing a modular overhaul on a blower. While removing magnetic rods for the stretch rod, the employee was placing the second rod on the blower platform. The rods were magnetically attracted to one another, and the employee's left third and fourth fingers were pinched between them. She was hospitalized and sustained a partial amputation.

An employee was conducting maintenance on the bag in box (BIB) palletizer machine when the motor chain jumped, pulling his right index finger into the chain drive attached to the motor and causing a fingertip amputation.

An employee was clearing a piece of tape from the tape dispenser when their hand contacted the cutting implement on the dispenser. The employee suffered the amputation of their right index finger.

An employee was troubleshooting a bottle machine because the nozzle was stuck in the up position. The employee went to check for pre-blow air and the nozzle dropped back down to its starting position, catching his right index fingertip against the mold base. The employee sustained a fingertip amputation to the first digit.

An employee was attempting to clear a bottle jam when they were pinned to the wall by the arm of a robotic bottle stacker. The employee sustained an abdominal injury that required hospitalization. The machine was not guarded at the time.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
