An employee was preparing to load a truck. They opened the dock door and were putting the dock plate in place, using their foot to push the plate down. The employee tripped and their right hand became caught in the hinge of the dock plate. Their middle and ring fingertips were amputated.

An employee was operating a trim die machine and the machine amputated 75% of the employee's left pinky finger.

On February 8, 2018, an employee was cleaning out the bed of a die cast machine's C-frame while a die setter was changing the machine's piston. The employee attempted to remove something from the machine just as the die setter activated it. His right ring finger was then caught between the platen and the sleeve ring, amputating his fingertip.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

An employee was adjusting the oscillator pressure injector on a die-cast machine when his right thumb contacted the adjustment handle. The machine oscillated and closed on his right thumb. The tip of the thumb was pinched and amputated between the die and the handle.

An employee in the melt department was getting ready to put a recirculation pump into the pump housing area. He first put the pump in a cabinet to warm it up to avoid thermal shock. It shifted and the cold refractory contacted the hot metal. Molten metal contacted the employee, resulting in third-degree burns between their knee(s) and ankle(s). The employee was wearing some personal protective equipment (PPE) at the time but this was breached by the molten metal.

On May 15, 2025, an employee was assisting with troubleshooting an issue with a die in a die casting machine. While unjamming the machine, the employee's finger was caught between a section of the die and a wooden spacer. The employee's left middle fingertip was amputated.

An employee was checking the die casting machine when the cavity access door closed behind him and the employee sustained eight fractured ribs. The employee was hospitalized.

An employee was using a vertical band saw to cut a piece of square metal tubing when their left hand contacted the saw blade and their middle finger was amputated.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
