The injured employee and employee 2 were lifting a filter housing (air bonnet) in preparation for a motor lift and removal at a client site. The filter housing shifted and the injured employee s left middle finger was caught between the filter housing and the concrete floor. The finger was partially amputated.

An employee had set down an electrical breaker box inside a motor control center he was preparing to replace the breaker in. He turned to walk away and an electrical arc flashed out of the open door of the cabinet, burning his palms and wrist and causing a slight burn to his right cheek.
