An employee was cleaning rotating parts on a print press using a rag. The rag was caught in a pinch point and her hand was pulled into the machine, resulting in a right middle fingertip amputation.

An employee and an assistant were disassembling two weldments that were approximately 6-feet tall, 3-feet wide, and 550 pounds. The weldments were held together by a connecting beam and tie bar. The first weldment was top strapped with an overhead crane to support the assembly and the second weldment was left free-standing. The employee removed the connecting beam and turned to walk away. One of the weldments tipped over onto the employee, causing them to fall to the ground and become pinned beneath the weldment. The employee sustained a compound fracture of the left leg and bruising to their torso and arm.
