Final narrative
An employee was assembling equipment. He was making a controlled adjustment to a pneumatic cylinder by slowly adding air pressure. As the pressure built, one of the safety latches dis-engaged, causing the cylinder to move suddenly and crush the employee's left middle finger against a mounting bracket. The employee's fingertip was partially amputated.

An employee was lifting a machine frame off of a welding bench and lowering it down to the ground. His fingers were under the frame to shift it when the frame fell and pinched his fingers, resulting in a fingertip amputation.
