An employee was assembling equipment. He was making a controlled adjustment to a pneumatic cylinder by slowly adding air pressure. As the pressure built, one of the safety latches dis-engaged, causing the cylinder to move suddenly and crush the employee's left middle finger against a mounting bracket. The employee's fingertip was partially amputated.

An employee was lifting a machine frame off of a welding bench and lowering it down to the ground. His fingers were under the frame to shift it when the frame fell and pinched his fingers, resulting in a fingertip amputation.

An employee was unloading a bundle of steel angle iron from a parked trailer in the shop delivery area using an overhead bridge crane. He was using the crane's hand pendant controller to control the crane's operation to lift the bundle of steel off the flatbed trailer and to lower it next to the trailer. At some point during the lowering operation, the wrong button was pushed on the pendant controller and the load swung and struck him in the chest while he was standing between the trailer and the swinging load. He suffered a punctured lung and broken rib(s).

This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. An employee and an assistant were disassembling two weldments that were approximately 6-feet tall, 3-feet wide, and 550 pounds. The weldments were held together by a connecting beam and tie bar. The first weldment was top strapped with an overhead crane to support the assembly and the second weldment was left free-standing. The employee removed the connecting beam and turned to walk away. One of the weldments tipped over onto the employee, causing them to fall to the ground and become pinned beneath the weldment. The employee sustained a compound fracture of the left leg and bruising to their torso and arm.
