An employee was preparing a boiler for curtailment (switching boiler from natural gas to a liquid fuel source). The employee had just removed the plug at the end of the boiler in preparation for the atomizing fuel oil gun to be inserted. When the plug was removed a flame event occurred engulfing the employee's face, neck, chest, and arms. The employee suffered second-degree burns to the face, arms, and hands.

An employee pulled out a bucket of hot oil from under the fryer. The employee then stood on the table to clean the back wall. He stepped down into the bucket of hot oil, resulting in third-degree burns to his left leg.

Ajinomoto Health and Nutrition North America Inc.

An employee was performing maintenance under a kettle. When he removed a tri-clamp on a transfer line, the hot fat and broth material in the kettle poured out onto his arm. The employee was hospitalized with burns.

After completing a run with an oil distributor truck, an employee was working to return (suction) the hot oil to the tank of the truck. When the employee opened one of the valves, hot oil (approximately 385 degrees) sprayed their face, resulting in first-, second-, and third-degree burns.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

Boise Cascade Wood Products, LLC

An employee was using a water hose to clean debris out of the outfeed of a log conditioning vat. Water began entering the vat from the adjoining vat through a void in the separation wall at the infeed of the vats. As the employee was exiting the vat he had been working in using the access opening at the outfeed, hot water exiting the access opening entered the top of his protective hip wader. It pooled at the bottom of the wader and burned his left foot and ankle.

An employee was opening a pig trap receiver during pigging operations when they were struck by metal grating. The employee sustained a right leg fracture.

An employee was climbing down a ladder when it shifted, pinching the employee's little finger against the overhead water pipes and steel support braces. The finger was severely lacerated, resulting in an amputation below the knuckle.

An employee was attempting to rig an 18' in diameter HDPE pipe. The employee was placing a nylon sling around the pipe when the pipe shifted unexpectedly. The employee's left pinky and ring fingers were pinched, resulting in an amputation of the ring finger to the second knuckle.

Norbert E Mitchell Company

Upon completion of his delivery at a customer's location, a driver returned to the truck, placed his feet on the driver's side step, reached into the cab and turned off the power take off. The truck began to roll. The driver tried to enter the cab to apply the brakes, but just before being able to stop the truck, the right front wheel exited the pavement causing the truck to roll onto its right side. The driver went over the truck landing onto the driveway below. The vehicle continued to roll over, crushing the left side on a rock. The truck came to rest with the damaged step straddling the driver's right leg, fracturing his right ankle.

An employee collected a sample cylinder of gas and was placing the cylinder into a weather guard job box that is used to organize the sample cylinders. The job box drawer closed on his left hand, amputating his left ring finger at the first joint, right below the nail.

An employee was troubleshooting the control setup for a variable air volume box above a ceiling. The employee's hand came into contact with an exposed, energized 277-volt wire that had loosened from its terminal on the box's assembly. He was shocked and fell off a ladder. His left hand passed across the ceiling grid and was lacerated. He also suffered an electrical burn to the right hand and was hospitalized.

An employee was assembling a baker type scaffold. He was beginning to transverse down the scaffold, approximately six feet off the ground, when the scaffold tipped over and he fell. The employee was hospitalized with a fractured femur.

An employee was working on an exhaust fan on the roof when he tripped on a wire and fell to the ground. The employee sustained scrapes on his elbow, and a fractured right knee with a torn ACL. The employee was hospitalized.
