Final narrative
An employee was rigging when their left little finger became caught between a bearing and a shaft flange resulting in amputation.
On 10/24/2021, at approximately 8:05 a.m., an employee was climbing scaffolding on board a ship when he slipped and fell approximately 18 feet to the flight deck below, resulting in a fractured leg.
An employee was operating a magnetic-mounted drill to drill one-inch holes in a stainless steel I-beam. As the employee was using a fox-tail style brush to sweep metal shavings off the I-beam his right middle finger was caught in the rotating drill bit. His right middle finger was degloved and two other fingers were fractured. He also sustained a fractured wrist. The employee was hospitalized.
An employee had been sand blasting and removed a valve pipe plug when it became clogged. As the employee was removing material, air bled out of the system and caused the valve piston to shut and amputate the employee's left index fingertip.
