An employee was positioning metal to be cut by a shear. The shear activated and the clamps pressed down on the employee's hands. Three of the employee's fingers were amputated.

An employee was assisting another employee to weld and place a flange onto the body assembly. After aligning the flange for placement, the employee went to pull it into place when their right index finger became caught between the flange and the flange web, resulting in a fingertip amputation.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.
