An employee in the melt department was getting ready to put a recirculation pump into the pump housing area. He first put the pump in a cabinet to warm it up to avoid thermal shock. It shifted and the cold refractory contacted the hot metal. Molten metal contacted the employee, resulting in third-degree burns between their knee(s) and ankle(s). The employee was wearing some personal protective equipment (PPE) at the time but this was breached by the molten metal.

An employee was sliding a steel work table when the table caught on an uneven surface and tipped over onto his left leg causing a fracture.

An employee entered the protective cell and was removing the steel liners from the die. The employee stepped up to the die, placing his left hand on the die surface to steady himself. The die core slide moved forward and the employee's fingers were caught between the core slide and the core guide plate, resulting in partial amputations to the left middle and ring fingers.

An employee was preparing a machine for setup and leaned forward to clean off an area. As he was backing out he misstepped over a hose, lost balance, and fell backward into the gravity slide table striking the left side of his back and head. The employee sustained a back contusion and a laceration (approximately 1 inch) to the head.

An employee was working on a running lathe. He was attempting to advance the ring on the well to make it flush with another component. When he attempted to mark the steel with a marker, the long finger of his left hand was caught between the gap in the ring, resulting in a fingertip amputation.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

A temporary employee turned off a planer machine, opened the housing, and began clearing a jam. The machine's spinning blade caught his right hand and degloved the index finger.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.

The machine was not locked out/tagged out at the time.

An employee was adjusting the oscillator pressure injector on a die-cast machine when his right thumb contacted the adjustment handle. The machine oscillated and closed on his right thumb. The tip of the thumb was pinched and amputated between the die and the handle.

On May 15, 2025, an employee was assisting with troubleshooting an issue with a die in a die casting machine. While unjamming the machine, the employee's finger was caught between a section of the die and a wooden spacer. The employee's left middle fingertip was amputated.

An employee was checking the die casting machine when the cavity access door closed behind him and the employee sustained eight fractured ribs. The employee was hospitalized.

An employee was using a vertical band saw to cut a piece of square metal tubing when their left hand contacted the saw blade and their middle finger was amputated.

The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.
