An employee was using a handheld cordless electric torque wrench. The tool's reaction arm came into contact with the employee's left hand, causing an amputation to the middle finger above the first knuckle.

An employee was using a remote control to operate a rail car mover to attach and relocate a subway car when their left hand was crushed between the rail car mover and the subway coupler, resulting in an amputation to the left hand.

On April 24, 2024, at about 8:30 AM, an employee was moving a die on a roller bed when their left middle and ring fingers became pinched between a guide wheel and the die. They sustained a crushing injury and lacerations to the fingers.

On July 21, 2022, an employee was compressing suspension springs in a hydraulic spring press machine. He had compressed two springs that morning and while compressing the third spring, suffered a fingertip amputation when the retaining bolt that holds the spring in its compressed position failed and the spring expanded.

An employee suffered a collapsed lung while lifting a jet ski hull that was raised on a hoist, pushing it sideways four feet to the press, and lowering it into place on the bonding machine. The employee was hospitalized.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.

An employee tripped over a concrete curb stop in a parking lot. The employee fell to the ground, landing on their left hip and breaking it.

An employee was cleaning mud out of the tracks of a mini excavator while the operator had the machine tilted up off the ground to one side. The tracks moved and the employee's left wrist was caught in the gears. The employee sustained puncture wounds to the wrist and two fractures.

An employee was loading a truck with a forklift. The truck drove away from the dock and the forklift fell between the dock and the truck. The employee was hospitalized with injuries to their head, back, and leg.

An employee was moving trucks in the yard. He was working between trucks when a back door swung open and caught their right hand against the frame of another truck. The employee sustained a crushing injury to a finger.

On August 21, 2025, a product development engineer was test riding an all-terrain vehicle (ATV) on a test track when the ATV flipped over. The employee sustained a fractured clavicle and three fractured ribs.

On June 9, 2025, an employee was walking in the yard and parking area. The employee stepped up from the yard/parking lot onto a patio with an approximate 6" rise at the exterior corner of the main office. After his second foot was planted on the patio surface, he lost his balance and fell backward to the concrete ground. The employee was hospitalized with a broken hip that required surgery and a laceration to the elbow.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was rappelling down a tree. When he was most of the way down, he fell on the ground and suffered a broken pelvis.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

An employee was cleaning out corn from a grain bin. The sweep auger in the bin was activated and caught his trouser leg. His leg was pulled into the auger, which injured his calf and caused leg fractures and lacerations.
