An employee was preparing a trailer for material removal and wrapping straps from the back of the trailer when he slipped on ice and fell. The employee was hospitalized for a tibia/fibula fracture.

An employee was delivering construction materials and was trying to loosen a tie down on the back of the truck. When the strap came loose it caused the ratchet bar to jolt forward pinching their left little finger between the ratchet bar and forks on the back of the truck and resulting in a fingertip amputation.
Employees were passing sheet rock from the first floor to the second floor. The injured employee stepped off of the stack of sheet rock and fell breaking the right leg.
An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.
