Employees were repairing a metal truss. As they lowered it to the ground, its weight shifted toward one of the employees. The employee's left index finger was caught between the edge of the truss and a steel-toe boot and was amputated between the first knuckle and the lower part of the nailbed. The employee was hospitalized.

CAPFORM, INC.

An employee was signaling a crane operator to hoist a form. As the crane lifted the load, the spreader bar chain pinched the employee's left middle finger. The finger was amputated.

Two employees were taking cross-braces off a shoring tower and handing them to a third employee at the base of the frame. They were working from the top down, and had removed the braces from the top two sections; as they began to remove the braces from the bottom section, the tower tipped over and landed on the employee on the ground. The employee suffered three broken vertebrae and three dislocated vertebrae in the back.

Three employees were attempting to relocate a shoring tower when the tower fell over and struck them. One of the employees received neck injuries and required stitches to the index and ring fingers. A second employee fractured the lower left leg and was hospitalized. The third employee received a contusion to the right hand.

An employee was cutting plywood with a powered hand saw when his left index finger touched the saw blade and was amputated.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

Thompson Hardwoods, Inc.

An employee and co-worker were working to unjam and move a 100-pound piece of crossed-up cant timber. The employee lost their grip and the timber fell on their right hand, resulting in crush injuries to the fingers.

Maxion Wheels LLC

An employee was working on a cushion ring (steel). As they lifted the cushion ring with both hands to flip it up, the ring slid down and the employee's right middle fingertip was crushed between the ring and the work table. The employee was hospitalized with a fracture, and their right middle fingertip was amputated.

HEXPOL Compounding PC, Inc.

Two employees were lifting a roll of steel poly bar (50 inches and approximately 150 pounds) from ground level to a height of 4 feet at the 66-inch rubber calendar. One employee dropped the roll, resulting in the poly bar contacting the injured employee's right knee. The employee was hospitalized with fractured right patella.

The injured employee was walking in front of the property by the front door as another employee was removing debris from the roof. The injured employee was struck by a 2 x 4 that was being cleared from the roof. He was knocked unconscious and sustained brain bleeding, as well as head and facial injuries, and required stitches.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was installing insulation in a wall when she tripped on a hole (12" deep) and fell, hitting her head on the concrete floor. The employee sustained fractures to her leg and wrist, and lacerations to her head. The employee was hospitalized.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.

An employee was operating a power drill when it slipped. The drill bit punctured the employee's hand and they were hospitalized.
