An employee was operating a forklift and picked up material from a shelf. While reversing the forklift, the employee's hand was crushed between a steel structure and the vehicle. The employee sustained a crush injury and laceration to the left hand.
An employee was welding on a 1,000-pound steel beam when a bolt on the turnbuckle support loosed and the steel beam tilted, striking the employee. The employee suffered a broken pelvis and was hospitalized.
