Final narrative
An employee was using a clamp fork truck to load a railcar with rolls of material. The employee exited the lift to put a mat between two of the rolls. The roll held by the fork truck's clamp (60 inches, weighing 3,000-3,500 pounds) dropped from the clamp onto his arm, covering it from hand to bicep. He sustained compartment syndrome, with blood clots, in the arm.
An employee was closing a boxcar door when his right middle finger was pinched between the door and the boxcar. The right middle finger was amputated to the first digit.
An employee was moving a 55-gallon drum of hydrofluoric acid onto a pallet on a forklift. The employee pushed the drum with his hands and knee. Approximately 1 hour later, he noticed his pant leg was wet and recognized the acid odor. He suffered 2nd degree burns to his knee cap and was hospitalized.
The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface.
