An employee was retrieving pallets from a conveyor feed using a stand-up forklift. While turning a corner on the forklift, the tip of his left foot struck a bollard, resulting in fractured toes.

The injured employee had been operating a forklift on a dock. He went to push another forklift with his forklift and a portion of the forklift entered the operator compartment. He sustained a fracture to his left foot/ankle.

The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
