An employee was making a right turn on an unloaded powered pallet jack and lost control. The employee jumped off the pallet jack and the pallet jack then pinched his right ankle against a pallet of finished goods, fracturing it.

An employee was moving a powered pallet jack when their ankle was pinned between the jack and a racking system resulting in a broken ankle and a ruptured Achilles tendon. The employee was hospitalized.

An employee was training on a lift when they struck a guardrail, resulting in a leg laceration.

An employee was accompanying a quality inspector who was conducting an annual quality audit on the beverage can filler production area. The quality inspector asked to see a strainer located on the piping housing. The employee proceeded to remove the strainer from the piping line. While attempting to remove the strainer, the strainer ejected from the piping and struck the employee above the left eye. The employee sustained a concussion and a laceration above the left eye.

An employee was transferring food into a cooler from a rack that was on a pallet jack. The employee was moving the pallet jack so the rack would be closer to the cooler. While getting off the pallet jack, the employee became caught between the pallet jack and the cooler. His right ankle was fractured.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

Two employees were removing a service lift platform from the back of a radar system. While handling the service lift platform, employee 2 was standing on the back of the radar and lost their footing, causing the service lift platform to shift and fall. The injured employee then fell from the top of the platform electronic subsystem (PES) to the floor, approximately 56 inches. The injured employee fractured his left elbow and left hip.

An employee fell from a loading dock to the ground 2-3 feet below. She suffered a compound fracture of the left hip and was hospitalized, requiring surgery.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was delivering beverages when he fell from the truck's liftgate to the ground. The employee sustained a fractured wrist, a dislocated shoulder, and bruised ribs.

An employee was performing maintenance under a van. The van slipped off the jack and struck the employee, who suffered broken ribs in the left upper torso. The employee was hospitalized.

During scheduled preventative maintenance, an employee was searching for vertical bagger shafts. Upon locating a box thought to contain new shafts, the employee removed two high-powered magnets from bubble wrap. The magnets forcefully attracted to each other and amputated the employee's fingertip.

An employee was walking beside a pallet jack while operating it and became pinned between the pallet jack and some pallets. The employee sustained a fractured left leg.

An employee was replenishing product in a warehouse aisle. They collapsed and fell onto their back, striking their head on the concrete and suffering a laceration to the back of their head and injuries requiring hospitalization and brain surgery.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.

An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.

An employee lost his footing while climbing an inclined ladder. He stumbled down one step and was able to stabilize himself by holding the railing and twisting his body, but he suffered an internal abdominal injury and was hospitalized.

An employee was manipulating dies on a brake press. The press caught and crushed the employee's right forearm, resulting in hospitalization.
