An employee was pushing a trailer that was resting on a sawhorse back into the weld area. His hands were on the sawhorse while pushing. The sawhorse stopped moving and the trailer shifted catching his hand in a pinch point between the trailer and the sawhorse. The employee sustained a partial amputation of the right little finger.

Employees were installing a removable jack onto the front of a trailer. The injured employee was aligning the bolt holes as the jack was being pushed into position. As the jack slid into place, his right index fingertip was amputated.

An employee was in the assembly warehouse taking a count of palletized product. Fender backs (raw material used in making finished product) slid off a pallet onto the employee's lower left leg. The employee sustained a fractured fibula and a dislocated ankle.

An employee was installing a cylinder underneath a trailer when a bolt on the conveyor came off and the trailer's back end came down and struck the employee. The employee suffered a fractured skull and broken ribs.

An employee accidentally shot himself in the right thigh with a pneumatic nail gun and was hospitalized.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was working on a production line. The employee went to retrieve parts that came out of the parts holder, and their left hand became caught between a chain and a sprocket. Three fingers on the hand were amputated.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a pad printer. The employee's finger was caught in the printer and they suffered a fingertip amputation. The part of the printer that caught the employee's finger was unguarded at the time.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was assisting with rigging a magnet to an overhead crane in order to lift a sheet of steel. The employee's right thumb was between the crane's hook and the chain passing through the hook, and when the crane was raised, the distal end of their thumb was crushed. Part of the employee's right thumb was medically amputated.

Two employees were working together to insert a pin into a hydraulic cylinder. One employee was holding the cylinder in place to align it with the pinhole, while the injured employee was inserting the pin. The injured employee was aligning the pin and moved to determine whether the hole alignment was too high or low. At that moment, the crowbar being used to assist with alignment slipped and his left index fingertip was pinched between the cylinder and the housing, resulting in an amputation just below the fingernail.

An employee was hoisting an axle in the buggy department when one side of the sling came loose, causing the axle to tip. The sling hook raised up from the weight transfer, causing the employee's left little finger to be caught by the rigidness of the hook. The fingertip was amputated.

An employee was using a circular saw to cut the cap off an aluminum bulkhead of a trailer that was being manufactured. The saw kicked, lacerating his hand and resulting in the amputation of his left middle and ring fingertips.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
