An employee was working on a project to remove the shafts of propellers and was working from a scaffold 15-18 feet from the ground. While working, one of the scaffold planks came loose and they fell through the scaffold to the lower level. The employee broke two ribs while trying to catch themself and fractured their heel from the impact of the fall.

On September 5, 2024, an employee was working from a ladder and aligning the top plate on a module when he lost his balance and fell 12-15 feet from the ladder through the opening of the module to the floor. The employee was hospitalized with a laceration on his head and three fractured ribs.
An employee was using a portable ladder to position and tack-weld steel panels onto sections of a ship. A panel was suspended from an overhead crane, and the employee was positioning it.
