An employee was descending a 10-foot A-frame ladder. The ladder came out from under the employee when he was about 5 to 6 feet above ground level. He fell, landed on the left side of his body, and sustained fractures to four ribs, his pelvis, and hip socket, and sustained injuries on the left side of his body. The employee was hospitalized.

An employee was handling a small platform ladder with three steps and two wheels in the front. The employee was found on the floor underneath the ladder. She was hospitalized with multiple fractures to the sacrum.

An employee was on a ladder four feet above the ground to install insulation. The ladder began to slide and the employee jumped to the shop floor, breaking his left tibia and fibula at the ankle. He was hospitalized, requiring surgery.

An employee's leg went through a part of the floor that was missing a floor tile and he sustained a dislocated shoulder while catching himself.

On July 21, 2025, an employee was working from a 10-foot step ladder while installing a 3-by-5-foot glass window pane on the outside of a new home under construction. The window fell from the frame and the employee went to catch it when the ladder slipped. The employee fell 5 feet to the ground and landed on small pieces of concrete left over from the foundation pour. The employee was hospitalized with a dislocated elbow and fractured wrist that required surgery.

An employee was on an extension ladder descending from a roof when the ladder collapsed and lowered. The employee fell approximately 20 feet to the ground, resulting in fractures to their ribs and left wrist.

The injured employee was acting as a spotter for another employee that was using a forklift to move crane plates. The injured employee went to walk on top of staged bracing poles that had been stacked. The top layer of bracing shifted when stepped on, causing the employee to lose balance and fall. During the fall, one of the bracing poles dislodged and landed on the employee s left leg, causing a compound fracture.

An employee was working to remove an existing window from a customer's home. The employee was descending an extension ladder when they fell backward onto a concrete patio. The employee sustained head trauma and fractures to their shoulder, ribs, and a vertebra.

An employee was painting the exterior of a house when they fell approximately 12 feet from a ladder. They landed on the ground and sustained a fractured right ankle.

An employee climbed a ladder onto a roof, then stepped from new roof sheathing onto a tarped section of old roof, and fell to the ground. The employee suffered fractures to their pelvis, vertebrae, and the left wrist. The employee was hospitalized.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
