An employee was putting a screw into position on a production part to weld with a spot welder. The spot welder tip crushed the employee's left thumb tip against the part and the injury required surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.
