A two-person underground electrical crew had just finished installing an empty electrical conduit and were completing final tasks for the day. These tasks included measuring needed wire lengths for installation into the new conduit and backfilling completed excavations. The injured employee was using a measuring wheel to measure from a tie-in hole to a nearby pedestal and the other employee was using a mini excavator to backfill the area around a newly installed pedestal. The excavator backed into the injured employee's lower left leg, breaking it.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.

An employee was working to assess the operation of a high-pressure water nozzle. The water system was activated and the employee was cut by high pressure water on his left hand where the palm meets the wrist. The employee was hospitalized.

An employee was sandblasting paint from a water tower when the blasting media sprayed across their forearm, resulting in an elbow injury that required hospitalization.

An employee was walking when he tripped on a small mound of dirt, fell, and sustained a fractured leg.

An employee was polishing the center of a try bar on a lathe when they sustained an avulsion of the right forearm that required surgery.

An employee was operating a dump truck when the trailer door swung and pinched the employee's right middle fingertip underneath the nailbed, resulting in an amputation.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
