Final narrative
An operator was removing a pipe from the forming machine. When the gate arm of the machine returned to its home position, it caught the end of the pipe, causing it to swing and strike the injured employee in the jaw. The injured employee sustained a fractured left jaw and neck injuries.
On April 30, 2024, at approximately 6:15 PM, an employee was guiding pipe into a metal bending machine and caught their right index finger in the roller area, resulting in an amputation of the distal digit.
