An employee climbed a ladder to put the cover back on the motor of a high-speed overhead door. He needed to duck his head to get around the track to perform work on the motor. When third-party employees walked in front of the door on the opposite side, they triggered the motion sensor and the overhead door began to open. As the overhead door opened, it struck the injured employee in the left orbital socket and knocked him off the ladder. He fell six feet to the ground below and sustained a fracture to his forearm just below the elbow and a bruised left hip.

An employee was removing the safety strut on a dock leveler when the clamp on the crane slipped, allowing the dock leveler to fall on the employee and drive him to the ground. He cut his ear and broke his leg.

Buckeye Surface Maintenance

An employee was operating an asphalt grinding machine when a branch got in the way of the machine. He crawled on top of the machine to cut the branch but it snapped back and knocked him off the equipment. He fell approximately 10-12 feet to the ground below, resulting in fractures to both wrists and forearms, and a left knee fracture.

Rough & Ready of Central Florida Inc

A temporary employee was on a ladder while cutting conduit on a new building using a reciprocating saw. They sustained an electrical shock from a live electrical cable. The employee fell backward approximately 6 feet to the ground, resulting in hospitalization with a fractured heel that required surgery.

RIGGINS-MORELAND ENGINEERING INC.

An employee was working on the ninth position of a 12-foot ladder, installing a new light fixture in an existing building. The employee sustained an electrical shock, lost his balance on the ladder, and fell. He sustained a fracture of the lower back and bruises on his hand and foot. The fracture required surgery.

Industrial Fabrics, Inc.

At approximately 10:00 AM, employees were conducting asphalt loading operations. While securing a hose atop a truck, an employee encountered a malfunction involving a plugged hose. Pressure built up within the hose and suddenly released, causing the hose to fly off the truck. The hose contacted two employees and both employees fell approximately 7 feet. Employee 1 was on top of the truck and sustained three fractured ribs and a head contusion. Employee 1 was hospitalized. Employee 2 was climbing a ladder on the truck and sustained a fractured right leg. Employee 2 was not hospitalized.

Sentry Electric, Inc. of Central Florida

An employee was working from a ladder, installing light fixtures. The employee was shocked and fell from the ladder to the floor about 6 feet below, striking their head and back. The employee was hospitalized with a laceration to the head and broken vertebrae.

Stevenson Crane Service, Inc.

A crew was relocating a dumpster (containing approximately 200 pounds of demolished wire) to access the uninterruptible power supply (UPS) units they were servicing. During movement of the dumpster, a section of carpet tile dislodged from the floor, causing an employee to lose his footing, slip, and fall. He landed on his right wrist, resulting in a fracture.

An employee was cutting a board using a circular saw. The saw pinched, kicked back, and cut the employee's left ring and little fingers. The employee suffered an amputation to the little finger.

Severo Ortiz Construction LLC

An employee was using an aluminum extension ladder to remove gutters at a height of approximately 20 feet. As they were removing the gutters, part of the gutter collapsed and struck the ladder, causing it to move sideways. The employee fell from the ladder and sustained fractures to ribs on the left side and their right forearm.

Fence Repair Company

An employee was using an electric saw to cut a piece of wood for a fence. The saw slipped out of his hand and lacerated his left leg. He was hospitalized.

Beth-Allen Scaffold & Equipment

An employee was erecting a scaffold. A side bracket was not secure. The employee stepped on the bracket to test it and fell 18 feet to the concrete floor resulting in fractures to the ankles and femurs. He was not using fall protection equipment.

Omya Inc.

An employee was working to remove a product blockage from a piece of equipment. The employee was struck by a metal bar near the deck of the pin mixer equipment and sustained blunt force trauma to the torso.

John Deere Coffeyville Works

An employee was moving a rack onto a lower conveyor in the south heat treat area when their finger got caught between the part tray and the rollers, resulting in a fingertip fracture and amputation.

Haven Steel Products Inc

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.

Vestas-American Wind Tech

Two employees were removing a circuit breaker. The injured employee was using wrenches in the removal process. One of the wrenches contacted an adjacent door, resulting in an arc flash. The employee suffered burns to their neck, face, and ear, as well as momentary vision loss due to the flash.

Alliance Ag and Grain LLC

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.
