An employee was using an ultrasonic welding machine to make filters when the anvil came down and pinned her left index finger against a metal bar. The employee sustained a severe laceration to the finger and probable nerve damage requiring surgery. The employee was hospitalized.

On Friday November 4, 2016, at approximately 5:30 a.m., a temporary employee was working with a coworker to hand feed fabric into a fabric cutter machine. At some point during the operation, fabric entangled the employee's hand and pulled it into the point of operation. The employee suffered a partial right ring fingertip amputation and lacerations to the right middle and pinky fingers.
An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.
