Final narrative
An employee was changing an air filter on heavy construction equipment when a truck of fill dirt backed into the door of the equipment, pinning the employee between the door and the mechanical parts of the equipment. The employee sustained fractured and dislocated ribs and fractures to their clavicle and sternum.

When they were finished, the forklift started moving the rack out of the area and a piece of material snagged one of the rack legs, causing the rack to tilt backward.
