An employee was assisting with rigging up the plant with shackles to move it out of the shop when their right thumb became caught in the chain, resulting in amputation to the first knuckle.
A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing.
