An employee was working on a seat assembly line at the rods and dials station. He finished the rods and dials and had the seat turned and was lowering it back onto the assembly line. His left thumb became caught in the trolley, resulting in partial amputation through the phalanx.

An employee was running a CNC tube bender. The bending clamp came in to grab the tube, and the employee's left index finger became caught between the clamp and the tube while it was compressed. The employee's fingertip was amputated.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a pad printer. The employee's finger was caught in the printer and they suffered a fingertip amputation. The part of the printer that caught the employee's finger was unguarded at the time.

An employee was moving a standup forklift backward toward the glue machine when the forklift contacted the cabinet. The machine then tipped backward, causing the bottom feet on the backside to collapse. The cabinet fell forward onto the forklift where the employee was standing. His right ankle was trapped between the cabinet and the forklift. He sustained a crushing injury to the ankle.

On May 6, 2025, an employee dismounted a forklift to place a box on a pallet. Another forklift reversed into the employee's parked forklift, causing it to move forward and run over their left foot. The employee was hospitalized with a crushed and fractured foot.

On May 5, 2025, an employee was using the office paper cutter to trim hook and loop fasteners when they sustained an amputation of their left thumb tip without bone loss.

On April 28, 2025, an employee was reinstalling a fire extinguisher on a railing that runs along the wall. The fire extinguisher fell and when the employee went to catch it, his left little finger was caught between the extinguisher and the railing, resulting in amputation of the fingertip.

An employee was walking in the hallway when they contacted another employee that was going in the other direction. The injured employee fell against a wall and then to the ground, resulting in a fractured right hip.

An employee was working to remove a product blockage from a piece of equipment. The employee was struck by a metal bar near the deck of the pin mixer equipment and sustained blunt force trauma to the torso.

John Deere Coffeyville Works

An employee was moving a rack onto a lower conveyor in the south heat treat area when their finger got caught between the part tray and the rollers, resulting in a fingertip fracture and amputation.

Haven Steel Products Inc

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.

Two employees were removing a circuit breaker. The injured employee was using wrenches in the removal process. One of the wrenches contacted an adjacent door, resulting in an arc flash. The employee suffered burns to their neck, face, and ear, as well as momentary vision loss due to the flash.

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.
