An employee was removing a tube from a mixer pump when the pump shifted and crushed the employee's finger between the plunger and the stop, resulting in a fingertip amputation.
An employee was rigging depleted gas bottles to be removed from a metal dumpster. A forklift's forks came down to retrieve the bottles. They caught his right middle finger against the rim of the dumpster, causing the amputation of the fingertip.
On April 4, 2022, at 9:45 a.m., an employee and co-workers were hoisting a fire pump via chain falls through a handrail onboard a ship. The fire pump electrical box got caught on the handrail section and broke free under tension, catching the employee's left thumb between the fire pump and the handrail.
