An employee was scraping dried glue from the top lid of a hot glue pot. His left hand slipped and came into contact with the hot glue. He then suffered burns to his hands and fingers as he tried to remove the glue. He was hospitalized.

An employee was working in the rendering department, being trained to clean-in-place (CIP) on equipment. A back-pressure valve was being removed to be replaced. When the employee removed a clamp holding the piping, hot water spilled onto the employee, who suffered second-degree burns to the back of the left hand, the right arm, and the abdominal area.

An employee was operating a machine in the rendering department and noticed a belt was moving slowly. He was working to make the belt go faster when the belt pulled his left hand and the pulley partially amputated his ring fingertip.

An employee was blowing out hot tallow when the tank overflowed and the employee was hospitalized with second-degree burns to his left arm and elbow.

An employee was carrying plastic wrappers across the harvest floor when his glasses became fogged and impaired his vision. He tripped over the corner of a pallet and fell to the ground, striking his head on a conveyor shield (framework for a conveyor belt). He sustained pain to the head, neck, face, bilateral upper extremities and lower back, as well as weakness and facial lacerations.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was using a de-clipping machine to cut the metal clips off of sausage sticks. As the employee put a link of sausages into the machine, their hand slipped into the point of operation. A piston pinched the employee's left index finger, causing a partial amputation.

An employee was operating a small tabletop grinder in a food safety quality assurance lab. She was stuffing meat down the grinder's chute when the grinder caught her right hand. Its auger caused an amputation to her middle finger and a partial amputation to her index finger.

An employee was cleaning a part on a pedestal grinder. The employee's middle finger contacted the abrasive wheel, resulting in a partial amputation.

A maintenance employee was removing an inspection plate from an industrial cooker. Hot liquid spilled onto him and burned his hands, arms, and torso area. He was hospitalized.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was rappelling down a tree. When he was most of the way down, he fell on the ground and suffered a broken pelvis.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

An employee was cleaning out corn from a grain bin. The sweep auger in the bin was activated and caught his trouser leg. His leg was pulled into the auger, which injured his calf and caused leg fractures and lacerations.
