On September 14, 2024, at 7:00 PM, employees were removing multi-purpose metal bars from an I-beam support of the new bridge deck. When the injured employee was lifting a bar, the bar slipped from his hands. He was catching the bar and his left ring and little fingers became caught between the knot of that bar and the I-beam support. He was hospitalized for surgery and medical amputation of his ring fingertip.

An employee was setting up a winch to adjust an internal cable/tendon when it jumped and the metal guide rod struck the employee on the right side of the cheek. The employee sustained a fractured jaw.

After unhooking a load, an employee backed away and signaled the tower crane to cable up. The rigging got caught on a metal I-beam. When it broke free, the rigging hook swung back and struck the employee on the forehead. The employee was hospitalized.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

An employee and co-worker were working to unjam and move a 100-pound piece of crossed-up cant timber. The employee lost their grip and the timber fell on their right hand, resulting in crush injuries to the fingers.

An employee was working on a cushion ring (steel). As they lifted the cushion ring with both hands to flip it up, the ring slid down and the employee's right middle fingertip was crushed between the ring and the work table. The employee was hospitalized with a fracture, and their right middle fingertip was amputated.

Two employees were lifting a roll of steel poly bar (50 inches and approximately 150 pounds) from ground level to a height of 4 feet at the 66-inch rubber calendar. One employee dropped the roll, resulting in the poly bar contacting the injured employee's right knee. The employee was hospitalized with fractured right patella.

The injured employee was walking in front of the property by the front door as another employee was removing debris from the roof. The injured employee was struck by a 2 x 4 that was being cleared from the roof. He was knocked unconscious and sustained brain bleeding, as well as head and facial injuries, and required stitches.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
