An employee was making a delivery. While working to offload a skid, the employee fell off the liftgate and landed on the ground, sustaining a fractured right ankle.

An employee was loading freight. They went to push the freight while standing on the forks of a forklift. The employee fell off the forks and struck the forks, sustaining broken ribs and a laceration behind an ear.

An employee was delivering freight when they were struck in the face by a metal crate. The employee then fell from the trailer liftgate to the ground, landing on their back. The employee's ribs were broken.

An employee was climbing down from a hostling tractor when he slipped and fell to the ground. He suffered a ruptured spleen.

An employee was delivering a freight crate to a facility. Upon moving the crate, a member of the crate came loose, striking the employee and causing him to fall to the ground. The employee sustained a head injury.

A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. The employee's left little finger was caught between the rig wrench and the embedded casing and was amputated above the top knuckle.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

An employee had been cleaning the work area. While lifting a saw, the employee's index finger was lacerated by the saw blade. A tendon was cut and required surgery.

An employee was installing a hydraulic extension cylinder in a tow truck when their leg was caught between the cylinder and the truck frame. The employee was hospitalized with a broken tibia, which required surgery.

An employee was sorting sucker rods. His right hand was caught in the area where the rod hits up against the frame and his index fingertip was amputated just below the nail.

An employee was inside a trailer, loading freight, when their leg was caught in shrink wrap. The employee fell, resulting in a fracture to their right thigh bone, which required surgery.

An employee was lowering a safe stack beam inside a trailer. The beam released, causing the employee to fall back and strike his head on the trailer floor. He was hospitalized with a brain bleed and a concussion.

An employee fell from a dock to ground level (less than 4 feet) and broke their tibia/knee.

An employee was rolling up the landing gear on his trailer when the handle began to unwind and struck his face, fracturing his jaw. The employee was hospitalized and required surgery.

On August 20, 2025, an employee was exiting a commercial tractor from the driver's side door when they fell to the ground, resulting in a fractured left ankle.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
