O & G Industries, Inc.

An employee was repairing a damaged load cover on a triaxle dump truck in which the right side of the load cover assembly was not raising up. The employee manually pushed up on the cylinder arm of the assembly, but the roller bar was stuck against two screws at the top of the tailgate. While standing on an 8-foot step ladder, the employee released the roller bar, causing it to rapidly spin due to spring tension inside the bar. The roller bar struck the employee's hand, crushing and lacerating his right index, middle, and ring fingers.

The employee pulled his gloved hand away and suffered a fingertip amputation.

On May 16, 2025, an employee was performing iron work. He was sliding a 3/4" x 2' x 4' bent plate across another plate to position it for installation. The bent plate crushed his left middle finger between the two steel plates. The distal bone was fractured and required a revision amputation.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.

An employee was operating a power drill when it slipped. The drill bit punctured the employee's hand and they were hospitalized.
