An employee was hanging parts on the powder paint line using hooks and eye bolts. An eye bolt came out of the threads and a part fell on the employee's right middle fingertip, resulting in amputation.

An employee was placing a rolling cart (12 feet by 4 feet) into a large oven. The employee's right hand was caught between the cart and the oven frame, resulting in the amputation of two of the employee's fingertips.
An employee was performing cleanup activities. As they reached to remove wood from the table saw, the wood broke, causing the employee's hand to strike the table saw. The employee sustained a fracture to the right index finger.
