Employee 1 was investigating a gas odor. While employee 1 was checking the equipment, a gas explosion occurred, resulting in a fire that burned the employee's face. Employee 2 entered the area and a second explosion occurred, causing burns to their face and arms. Employee 2 was hospitalized.

Shumard Corporation

During fiberglass work, an employee mixed a methyl ethyl ketone peroxide (MEKP) catalyst with a cobalt chemical that they thought was a dye. A chemical reaction occurred, resulting in an explosion. The employee sustained second-degree burns on his back and a blister on his right arm.

WVA Manufacturing, LLC

At about 7:30 p.m. on August 12, 2025, an employee was using a backhoe to excavate accumulated slag and debris from a furnace tap area and ladle pit. Residual water in the material had not fully evaporated; upon disturbance, it reacted violently, resulting in an explosion. The employee suffered first, second, and some third-degree steam burns, mainly to the arms and knees with additional burns to the hands and the back of the neck. The employee was hospitalized.

An employee was cutting glass for a custom windshield and was using denatured alcohol during the cutting process. The denatured alcohol caught fire and the flame traveled to a gallon-sized container of the liquid. The can exploded and the employee sustained second- and third-degree burns from the waist up.

Affiliated Fuel System, Inc

On April 18, 2025, an employee was cutting the lid off an empty 55-gallon drum using a plasma cutter. The drum originally contained a flammable liquid and residual liquid inside the drum ignited and exploded. The employee was hospitalized with burns to the face, chest, and arms.

Smart Building Technologies - A Fidelity Company

An employee was troubleshooting the control setup for a variable air volume box above a ceiling. The employee's hand came into contact with an exposed, energized 277-volt wire that had loosened from its terminal on the box's assembly. He was shocked and fell off a ladder. His left hand passed across the ceiling grid and was lacerated. He also suffered an electrical burn to the right hand and was hospitalized.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was standing on an A-frame ladder approximately 6 feet above the floor while he attached a suspended light fixture to the ceiling. The employee lost balance, and fell to the tiled floor below, sustaining blunt force trauma to his head.

Cupertino Electric, Inc.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

Swedberg Electric, LLC

The injured employee was performing routine maintenance on a fan belt/pulley. The injured employee and another person were moving the pulley back and forth manually to find a bearing issue. The injured employee's left ring finger got caught between the belt and the pulley wheel, resulting in a soft tissue amputation.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

APM TERMINALS PORT ELIZABETH

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.
