An employee was descending an A-frame ladder. He lost his balance and grabbed an exposed lightweight metal wall stud, which caused a severe laceration to his right middle finger as well as a tendon rupture. He was hospitalized, requiring surgery.

An employee was on a scissor lift installing sprinkler pipes. The employee was trying to get a better angle to install a pipe, left the lift basket, and fell approximately 20 feet to the ground. The employee was hospitalized for a broken left heel, right knee and right ankle.

WALT DISNEY PARKS AND RESORTS-US

An employee was cleaning the pretzel oven when the edge of a rack in the oven contacted their right thumb, resulting in a laceration that required hospitalization and surgery.

An employee was servicing an exterior mounted environmental control unit while on a 10-foot ladder. He pushed through a pre-made knockout hole on the panel. Upon bringing his arm down, a sharp area along the panel frame lacerated the top of his hand. The employee required surgery.

An employee was tightening a gas pipe with a wrench when the wrench slipped from his hand. The employee's arm then hit a metal cross-brace, resulting in a lacerated arm.

An employee bent down to turn on an air valve, lost their footing, and struck their hand on a pressure relief valve while trying to catch themselves. The employee suffered an amputation to their right index fingertip.

Brown's Tree Service, LLC

An employee was helping to hold a rope that was tied around a pine tree where dead branches were being removed. As a branch was removed, the tree swung, causing the employee holding onto the rope to swing, slide across the grass, and strike the bumper of a nearby truck. The employee sustained a lower leg fracture.

Stevenson Crane Service, Inc.

A crew was relocating a dumpster (containing approximately 200 pounds of demolished wire) to access the uninterruptible power supply (UPS) units they were servicing. During movement of the dumpster, a section of carpet tile dislodged from the floor, causing an employee to lose his footing, slip, and fall. He landed on his right wrist, resulting in a fracture.

An employee was cutting a board using a circular saw. The saw pinched, kicked back, and cut the employee's left ring and little fingers. The employee suffered an amputation to the little finger.

Severo Ortiz Construction LLC

An employee was using an aluminum extension ladder to remove gutters at a height of approximately 20 feet. As they were removing the gutters, part of the gutter collapsed and struck the ladder, causing it to move sideways. The employee fell from the ladder and sustained fractures to ribs on the left side and their right forearm.

An employee was using an electric saw to cut a piece of wood for a fence. The saw slipped out of his hand and lacerated his left leg. He was hospitalized.

An employee was erecting a scaffold. A side bracket was not secure. The employee stepped on the bracket to test it and fell 18 feet to the concrete floor resulting in fractures to the ankles and femurs. He was not using fall protection equipment.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.
