An employee was spotting two forklifts that were lowering a conveyor belt frame. One of the forklifts lowered farther than the other, creating tension in the conveyor belt. The belt sprung, causing the conveyor to fall 8 feet off the forklifts and strike the employee in the leg. The employee sustained a dislocated patella and ligament damage.
An employee was accessing the top of a river barge with a portable ladder. The employee fell from the ladder, suffering a broken bone in the shoulder area.
An employee was checking the gap between the feed rollers when his right hand was caught in the rollers. Multiple fingers on the right hand were amputated.
