An employee was troubleshooting a filling station that was overfilling tubes of product. The overflow on the crimpers was getting onto the outer tubing/packaging. The employee went to wipe product from the number three crimp and the machine cycled, catching the employee's right hand. The employee's right middle finger was amputated just above the first knuckle. A guard was in place at the time.

An employee was removing a blister from a machine on a manufacturing line. She suffered a partial amputation to her right middle fingertip.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

Sargento Foods Inc

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

Innovative Chemical Technologies

An employee was on a ladder disconnecting a 1-inch hose, known to have last carried acrylic acid that had been drained. When the hose was disconnected, residual acid dripped onto the employee's shoulder/arm area, causing a second-degree chemical burn.

Opta Turtle Creek

On October 1, 2025, at approximately 9:50 p.m., an employee was removing a sack of debris from a dust collector when four of his right-hand fingers were severed by a rotary valve. The employee was hospitalized and underwent surgery to amputate the four fingers at the knuckle.

Chemtrade Refinery Service Inc

An employee was unloading and depressurizing a railcar filled with sulfuric acid. The employee turned the valve to disconnect the hose when sulfuric acid released and sprayed upward via the standpipe. The employee was hospitalized burns.

On May 8, 2025, an employee developed a cough while cleaning up chemical sewage from the floor. He was hospitalized, having suffered an allergic reaction to chemical product vapors.

An employee was loading a magazine of tubes into a machine to fill. As they raised the door into position, the door dropped onto the employee's right hand and amputated their right index fingertip at the nailbed.

United States Postal Service

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.
