Two employees were using a power washer when they received a shock to their right hand and left foot. Both employees were hospitalized.

An employee was removing pita material from a stopped conveyor. A fixed metal guide and frame caught the employee's hand, causing a fingertip amputation. The employee was hospitalized.

An employee's left index finger was amputated while attempting to clear a piece of meat from a meat mixing machine.

An employee was injured when the stand-up forklift he was driving hit a parked forklift. One of the parked forklift's forks entered the cab, struck his foot, and cut his foot and leg. He was hospitalized.

A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. The employee's left little finger was caught between the rig wrench and the embedded casing and was amputated above the top knuckle.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

An employee had been cleaning the work area. While lifting a saw, the employee's index finger was lacerated by the saw blade. A tendon was cut and required surgery.

An employee was installing a hydraulic extension cylinder in a tow truck when their leg was caught between the cylinder and the truck frame. The employee was hospitalized with a broken tibia, which required surgery.

An employee was sorting sucker rods. His right hand was caught in the area where the rod hits up against the frame and his index fingertip was amputated just below the nail.

The depositor heads lowered and pinched the employee's left ring finger, resulting in an amputated fingertip. The whipped cream dispensers were not guarded at the time and the line was not locked out/tagged out.

During a line changeover, an employee was rinsing a depositor with water. The employee was working to remove a piece of chocolate, stuck in the machine's roller, when the roller's moving parts caught his middle finger. He suffered an amputation to the fingertip with bone loss.

An employee was emptying a trash can into a dumpster. He tripped while turning around, fell over a rail onto the concrete floor, and broke bones in his back and wrist.

During a changeover, an employee was wiping down the dough chunker machine and the chunker closed on his left hand. The employee sustained crushing/laceration injuries, requiring hospitalization and surgery.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
