An employee was unloading material stored in a crate in the bed of his pickup truck. He was repositioning the crate and pulling on it when the screws securing the wooden component failed, causing it to detach. The employee lost his balance and fell from the truck bed to the concrete. The employee sustained a hip fracture.
An employee was deburring a part on a CNC lathe when his glove became caught and his hand was pulled into the lathe, resulting in a right index finger amputation at the second knuckle.
