Federal OSHA recorded a severe workplace injury at Woodard & Curran, Inc., 3300 Panel Way Drive, SAINT CHARLES, MISSOURI 63301 on — Fractures , affecting the Nose and mouth.

An employee was going to visually inspect a wastewater lift station when a gust of wind blew the double hatch door (each door is 36" x 30") that they were holding up. The door fell and struck the employee in the back of their head, causing their face to strike the concrete and the steel edge of the lift station. The employee was hospitalized with a non-displaced fracture of the maxillary sinus.

An employee was attempting to open a valve next to an outdoor tank. The valve broke loose and the employee's left middle fingertip got caught between the flange and the tool he was using, resulting in a fingertip amputation.

An employee was prepared to deliver approximately 4,500 gallons of hydrofluoric acid and was exposed to chlorine gas. The employee sustained respiratory burns and was hospitalized.

An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.

A building engineer was inspecting a generator. As he closed the generator enclosure door, his right ring finger was caught between the door and the door frame. The fingertip was amputated.

An employee was replacing a water check valve at a water plant. He was lifting the valve with two hands when it slipped and contacted his right ring finger, resulting in a partial amputation without bone loss.

Del-Co Water Company

The injured employee was assisting with hitching a trailer to a truck. The employee was working to engage the lunette ring on the trailer with the pintle hook on the truck. A mini-excavator began driving onto the rear of the trailer. The added weight caused the rear of the trailer to lower and the front to rise, creating a pinch point between the trailer and the truck. The employee s left thumb was caught in the latch mechanism of the pintle hook. The thumb tip was partially amputated and required surgery.

An employee was spraying down an area by a conveyor he had been using. Moving parts (such as rollers and chains) inside a gear box amputated his right arm at the elbow. The employee was hospitalized.

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.
