An employee parked her truck but it started rolling. She tried to get back in the truck and then tried to get out again before the truck rolled into a river. She got caught between a tree and the truck door. The employee sustained multiple rib fractures and an abrasion to her shin that required a skin graft.

An employee was walking toward a home when the employee tripped, fell, and broke the left arm.

An employee was passing out fliers when she fainted due to the heat.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was working on a production line. The employee went to retrieve parts that came out of the parts holder, and their left hand became caught between a chain and a sprocket. Three fingers on the hand were amputated.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a pad printer. The employee's finger was caught in the printer and they suffered a fingertip amputation. The part of the printer that caught the employee's finger was unguarded at the time.

An employee noticed a polymer mixer pump was shaking and went to troubleshoot it. The employee was working to rotate the shaft of the mixer pump when the shaft started and grabbed his glove. He sustained a compound fracture to the left thumb.

An employee was closing a lift gate on a box truck. The gate slipped and pinched the employee's left ring finger, resulting in multiple fractures.

An employee was taking pictures of algae and went to use the ladder when they fell 10 feet onto the cement. The employee sustained a broken leg that required hospitalization.

Employees were lubricating a chain and sprocket on an entry gate to a treatment plant. One employee's ring and middle fingers were pulled into a control panel motor drive sprocket, which amputated both fingertips.

An employee was a passenger in a golf cart. He fell off and struck his head on the ground. He suffered a skull fracture and was hospitalized.

An employee was attaching a steel angle plate weighing (approximately 200 pounds) to the end of a structural member (5,000 pounds) and was supported by dunnage. The dunnage was a crane mat (11 inches tall) with a 4x6 board on top of it and it supported five identical members. The main piece was 30.75 feet long, 37.25 feet tall, and 1 food wide. As the employee slid the piece of metal on top of the member to begin attaching it, the entire member began to tip over. The employee went to stop the main piece from tipping over and it landed on his ankle, fracturing it.

An employee was preparing hot melted butter on a flat-top grill and placing it in small containers once it was melted. As she turned to set the container down, it slipped out of her hand and landed on the grill. Butter splashed up and hit her in the face. The employee suffered second-degree burns to her right eye and the right side of her head and face, as well as first-degree burns to her right forearm.

An employee pulled into the customer's driveway, parked, and exited the vehicle. He opened the sliding side door on the vehicle to retrieve a package. The customer began backing their vehicle out of their garage and it struck the employee in the back of his left leg. He was pinned him between the two vehicles, causing contusions and lacerations.

An employee reached into a hamper, picked up a package, and was hospitalized with a back injury that caused pain in his lower extremities.

On July 10, 2025, an employee tripped over a yellow pillar and fell to the floor. The employee was hospitalized with a fractured left femur.
