An employee was walking toward a door entry when he lost his footing on the edge of the sidewalk and fell to the ground, suffering multiple fractures to his hip joint.

An employee was at the dock delivering materials using a pallet jack. The wheel of the pallet jack fell into a gap between the dock plate and the truck. As the employee was pulling on it to get it out a part of the skid broke and the employee fell injuring his right leg.

Waiting for his truck tire to be repaired when the vendor's air compressor exploded, causing truck damage and shrapnel to strike the employee. He suffered cracked vertebrae and lacerations that required hospitalization.

An employee fell from the trailer of a transport truck while unloading material striking his head and shoulder resulting in a broken clavicle and concussion.

The switch truck rolled and pinned the employee against the container. The employee sustained a fractured pelvis, a ruptured spleen, and a possible spinal cord injury.

On October 16, 2025, an employee was entering a parking lot after completing their work shift when they were struck by a vehicle being operated by a customer. The employee fell backward from the impact and the back of their head struck the asphalt. The employee was hospitalized due to brain bleed and a skull fracture.

An employee was moving branches and other material when they were struck in the leg by the track of a skid-steer loader, resulting in an ankle and lower leg injury.

An employee was waiting for an operator to bring mail over to a mail sorting machine when she became pinned between the machine and a stack of pallets being pushed by a powered industrial truck (PIT). The employee suffered bruising and swelling on her hips, lower back, knees, and left side; a puncture wound to her left thigh from a machine screw; a crushed right hand with numbness and tingling; numbness to the left big toe; and a right wrist sprain.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

An employee was hooking up a set of 28-foot trailers, attaching a converter dolly to the lead trailer's pintle hook. He backed up the lead trailer and parked it a foot from the converter dolly, then lifted the converter dolly and rolled it to the lead trailer to set it onto the hook. The converter dolly kicked up, and the employee's left hand was caught between it and the trailer. He suffered a severe laceration to the palm between the ring finger and middle finger.

An employee was cleaning the windshield of his truck. When he stepped down from the truck, he lost balance and reached to catch himself using the side mirror. The impact fractured his left wrist. The employee was hospitalized and required surgery.

An employee was standing on the DOT bumper on the back of a standard 53-foot trailer. The employee lost his footing and his grip on the handle bar, and fell approximately 42 inches to the ground. The employee was hospitalized with a fractured left hip/femur.

A driver was making a delivery. As he was climbing onto his trailer to get boxes, the strap he was using to pull himself up broke. He fell from the back of the trailer to the ground, landing on his right leg and breaking his femur.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.
