Final narrative
An employee was seated at a press brake and was putting strips of steel (2-inches by 2-feet) into a brake press machine. The machine actuated and his right ring finger was crushed between the upper die and the steel material. The finger was amputated through the base of the nailbed.

An employee was loading a piece of round aluminum into a CNC auto-spinning machine. Their left thumb was crushed between the tail block, the aluminum part, and the mandrel. The distal tip of the thumb was amputated.
An employee was lifting a fan part with a jib crane to take it from vertical to horizontal. The part, a large hollow steel cylinder with a flange on each end, was attached with lifting lugs and two lifting straps on either side of it in and upside-down V coming down from the crane hook.
