An employee was moving a steel coil using an overhead crane and was setting it down when the coil struck other coils in a bin, causing those coils to shift. The employee was struck by the steel coils and sustained a fractured hand.

An employee was unloading 3' x 24' metal grating from a press machine and moving it to another location using an overhead gantry crane. The material shifted during the move and struck the employee's lower left leg resulting in a laceration to the calf and a broken lower leg bone.

An employee was welding a piece of grating at her work station when her shirt caught fire. She sustained burns to the left side of her torso.

An employee was clearing a jam in a press welding machine. The machine activated and the guard came down onto their arm and hand, resulting in fractures.

An employee was using a circular saw to cut aluminum grating. The saw caught and injured his left forearm. The arm was medically amputated.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was working on a production line. The employee went to retrieve parts that came out of the parts holder, and their left hand became caught between a chain and a sprocket. Three fingers on the hand were amputated.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a pad printer. The employee's finger was caught in the printer and they suffered a fingertip amputation. The part of the printer that caught the employee's finger was unguarded at the time.

A temporary employee was performing a two-person projection welding task to add a hinge reinforcement to a hollow steel door. He was positioning the hinge reinforcement while his supervisor held the door to maintain alignment. When the foot pedal was actuated, the electrode partially amputated the employee's left ring finger.

An employee was stacking bundles (12 feet long, 21 inches high) of aluminum extrusions (weighing 28 pounds each). After three bundles had been successfully stacked, the fourth tipped over when it was partially complete. It landed on the employee's right ankle/leg and broke the tibia. The employee was hospitalized.

An employee was adjusting the rollers on the mill feeder when their right thumb was crushed between two rollers, resulting in amputation of the thumb tip.

An employee was operating a core drill to bore through concrete at an active construction site. The equipment struck embedded rebar in the concrete, causing the handle to kick back and strike the employee's ribs. The employee was hospitalized with a fractured rib and a punctured lung.

An employee was loading pallets onto a truck and climbed down off a pallet after loosening straps to release the crane. The employee slipped off the flatbed truck and fell 4 feet to the dock plate/ground. The employee was hospitalized with a brain bleed and fractures to their T4, T5, and T6 vertebrae.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
